Return Customers Only – Pre-registration 

Returning Families simply access your MyProcare account at myprocare.com and select the Fall pre-registration option in the lower left hand corner of the screen to pre-register (pre-registration does not register your child, it only starts the process.  See below to complete the registration process)  If you have not yet created a Myprocare account, enter your e-mail address listed with SHADES to get started.

Your registration is not complete until you have been contacted by a SHADES staff member to complete and an orientation.  You will be given a deadline (typically 48 hours from the time of contact) to complete and submit orientation, paperwork, and applicable fees. Failure to adhere to the deadline will result in forfeiting your child space and you will be added to the end of the pre-registration waiting list. 

Many of the forms listed below are fillable PDF forms that may be saved and e-mailed or printed and submitted to your site location.  You must have Adobe Reader to open and complete these forms.  Click to download Adobe Reader.

Select your school.  Once you have completed pre-registration information, you will be contacted to complete an oreintation if space is available in the program.  You will be given a deadline (typically 48 hours from the time of contact) to complete and submit orientation, registration paperwork, and any applicalble fees.  Failure to complete the registration process by submitting all required items will forfeit your child’s space and you will be moved to the end of the pre-registration waiting list.  Once your account is set up, payments may be made by credit card through myprocare.com.  

Enrollment Paperwork

SHADES of Development provides a variety of services that match the needs of the communities we serve. Please select the site location appropriate for you. You may then either complete the paperwork as a fillable document, save and e-mail it to your site location, print and complete the paperwork or visit your site location to receive the paperwork. All SHADES participants must attend orientation, submit the requested paperwork and pay any applicable fees in order to complete the registration process. Your Site Director will call to set up an orientation if space is available.  Site contact information can be found under the location tab of this website.
